EMPAC Investor Form

• This is the updated form for members to fill out at membership events, district meetings, etc. It includes the incentives for new investors and investors who increase their contribution.

EMPAC Recruiter Form

• This is the form that EMPAC recruiters can fill out and submit. For every ten new investors or investors who increase their contribution by $5 or more, you will receive a $50 check. Submit form by email to EMPACChair@seanc.org.
